Service Dog Handler Application Guidelines

The process of becoming a MobilityDog Handler first requires attending a  Zoom Service Dog Handler Informational Session .

Then complete your MobilityDog Handler Application. In order to complete the application in the Google Form, you will need the following:

Check for $350 Application Fee sent to MobilityDog or credit card $362.  

Time to write about yourself, background, medical history and why you want a mobility service dog.

Time to fill in all six parts of the application: 

  1. Contact information and background.

  2. Physical Needs & Evaluations.

  3. Concerns. 

  4. Your Residence, Family, Lifestyle & Service Dog.

  5. Medical Information.

  6. Documentation Data & Agreements.

The estimated time to complete the application is one to three hours.

Please have ready contact names, addresses and phone numbers of yourself, family, emergency contacts, healthcare professionals, veterinarian and references.

You will also need uploadable photographs, videos and ID.

Remember, your application will only be complete when the following is completed and on file with MobilityDog :  

  1. Read the FAQ.

  2. Participated in a Handler informational Session. 

  3. Paid fee and completed the application form, including a copy of your photo ID.

  4. Provided a copy or link to your self-created video of you doing something you enjoy to info@mobilitydog.org.

  5. Completed Physician Form- FAX +1 (626) 628-3965.

  6. Completed Physical Therapist(s) Form- FAX +1 (626) 628-3965. 

  7. Completed Occupational Therapist(s) Form- FAX +1 (626) 628-3965. 

  8. 3 letters of recommendation, using the guidelines provided sent to MobilityDog (info@mobilitydog.org or FAX +1 (626) 6628-3965:

  • One from someone living with the Handler Applicant.*

  • Two recommendations from people living outside the Handler’s home.

  • Only one family member out of the three recommendations.

  • *If the Handler Applicant lives alone, please ask for a letter of recommendation from a neighbor or someone who sees you move daily.
